Before considering any weight loss medication, it's essential to prioritise healthy lifestyle changes. Incorporating a balanced diet and regular physical activity into your routine can be effective in achieving weight loss goals.

However, for some individuals, additional support may be needed. There are alternative medicated treatments available (see above) that can help with weight loss, in some circumstances. These are only available after a full medical consultation with a clinician. 

A healthy weight can mean having more energy, which in turn provides motivation to incorporate regular exercise into your day-to-day routine. This helps you to maintain that healthy weight and can also support positive mental health and wellbeing. 

A BMI in the healthy range for your height, gender, and age, along with a healthy waist size, may be associated with a lower risk of conditions like high blood pressure, heart disease, and type 2 diabetes, plus day-to-day benefits like less tiredness and joint pain. 

Making positive lifestyle changes can be beneficial to prepare for side effects. You can start increasing your light activity, such as walking or stretching, and change your eating habits by consuming smaller, slower meals, foods higher in protein and fibre, and avoiding greasy or heavy meals. It’s also essential to stay well-hydrated, so be sure to have water readily available to sip throughout the day. 

The most important thing you can do is to speak to a clinician as they can help you understand if the side effects you’re experiencing are common and check over any potential severe symptoms.

Losing weight fast isn’t the goal, but reaching the targets set with your clinician in a healthy, manageable way should be. The best way to maximise the chances of achieving your goals while taking weight loss medication is to combine it with positive lifestyle changes, such as regular exercise and a healthy, well-balanced diet.

A reduced appetite doesn’t automatically mean you’ll miss out on nutrients, but it can happen if you’re eating less or choosing only very light foods. Your appetite may change, but not your body's nutritional needs, so the key is to ensure your diet includes nutrient-dense foods high in protein, fibre, and healthy fats, alongside a variety of fruits and vegetables. The NHS recommends using your Body Mass Index or BMI as one possible indicator of a healthy weight range. The BMI is a score that’s calculated based on your height, weight, and age. The score for a healthy weight, depending on these factors, sits between 18.5 and 24.9. BMI is one measure used to assess suitability, but it is not a complete assessment of health. Your clinician will consider your BMI alongside other relevant clinical information. 

To get a better picture of your weight health, the NHS advises that you should also measure your waist size. This is because excess fat in your tummy area can mean a higher risk of health conditions like heart disease, type 2 diabetes, and stroke. It’s recommended that you may need to lose weight if you’re female and your waist size is over 31.5 inches or if you’re male, 37 inches.

The prescription-only injectable medicines approved in the UK as a weight management aid are known by brand names like Wegovy (semaglutide) and Mounjaro (tirzepatide). 

Weight loss injections can aid with weight loss because they contain medicines that regulate appetite and help people feel fuller for longer. They work by mimicking naturally occurring hormones that play a role in appetite regulation. Semaglutide (Wegovy) mimics a hormone called GLP-1, while tirzepatide (Mounjaro) mimics two hormones, GLP-1 and GIP. Both slow the rate at which food moves through the gut, which can prolong feelings of fullness and reduce the urge to eat as much or as often, reducing calorie intake and encouraging weight loss.

If opting for a private treatment, the cost of weight loss injections will be set by the clinical provider who prescribes them. This cost will vary depending on the brand of weight loss injection prescribed, and the strength of the medication.

Some weight loss injections are available on the NHS as a weight management option for certain patients, and in these instances, the cost will be the standardised NHS prescription fee (currently £9.90 per item) unless you are exempt from paying for your NHS prescriptions.

There is no single ‘best’ injection, or treatment of any kind, for weight loss. The most suitable weight loss treatment for you depends on your individual health profile, your BMI, and your clinician’s assessment of your consultation form.

When you stop taking your weight loss medication, the effect of the medication will gradually fade. Appetite and hunger may increase, and the feeling of fullness after eating may reduce. Gastric emptying returns to its previous rate, which can mean feeling less full after meals. 

This means the likelihood of putting the weight back on you have previously lost increases. One study from 2022 found that after 68 weeks of weight loss treatment, the average patient saw a drop of 15% in their body weight, but within one year from the end of their treatment, patients saw an average of two thirds of their prior weight loss returned. 

Some of the common and very common side effects of Mounjaro weight loss injections include:

These side effects are usually not severe. They are most common when first starting the medication but decrease over time in most patients. Reducing fatty foods, eating bland food, eating smaller portions, and staying hydrated may help manage some of the side effects such as nausea, diarrhoea and vomiting. 

The rare and severe Mounjaro side effects include:  

• Allergic reaction or anaphylaxis - If you experience this, stop taking the medication and contact emergency services for immediate help 

• Pancreatitis - If you experience severe abdominal pain, with or without vomiting, stop taking the medication and contact emergency services for immediate help

Mounjaro and Wegovy weight loss injections are prescription-only medicines that have been assessed by regulators for safety, quality, and effectiveness. They are not suitable for everyone and should only be used under clinical supervision. 

The safety of the medication depends on several factors, including having a consultation to ensure you're a suitable candidate, following the correct dosage schedule, and regular monitoring by a clinician. This gives you professional guidance and support should you need it, such as managing side effects or issues with your medication.  

However, the popularity of GLP-1 medications has increased, which has coincided with the increase of illegal and falsified versions of the medicine. These may be falsely advertised online and on social media. Taking unauthorised GLP-1 medication can cause serious or unexpected side effects and may interact dangerously with any other medication you’re taking.  

GLP-1 injections are only available on prescription and should only be taken under the supervision of a healthcare professional. Never buy them without a prescription or from unverified sources.  

You can take the maintenance dose agreed with your clinician for as long as it’s helping, as long as you’re not experiencing any side effects (or if any common side effects are being managed), and as long as it’s clinically safe. While using the medicine, it’s important that you keep in touch with your clinician or provider to stay on top of your progress or any issues you are having.    

If you experience an increase in weight, you should include this on your next consultation with a clinician for your repeat treatment so that they can review your dosage. If you wish to discuss stopping your maintenance dose, you should speak to your clinician, who will advise whether this is clinically appropriate for you. If so, they can work with you to develop a structured plan for stopping treatment and support you in maintaining your weight loss through longer-term lifestyle changes, including diet and physical activity.  

If you wish to restart treatment, you need to submit a consultation form. A clinician will review your full treatment and medical history to determine whether restarting medication is safe and appropriate. All prescriptions must align with current UK prescribing guidelines, which require a clinical need based on your present eligibility (e.g., BMI and health risks). Your clinician will ensure any decision prioritises your safety and long-term weight management goals.

Weight-loss tablets work in different ways depending on their active ingredient.  

Wegovy contains semaglutide, which is a glucagon-like peptide-1 (GLP-1) analogue. It acts as a receptor agonist, meaning it activates the GLP-1 receptor present in areas of the brain involved in appetite regulation. 

Foundayo contains orforglipron, which is a nonpeptide GLP-1 receptor agonist, meaning it also activates the GLP-1 receptor, helping to reduce hunger and food intake. 

Both semaglutide and orforglipron support weight loss by: 

• Reducing appetite 

• Helping you to feel fuller for longer after meals 

There are other non-prescription weight loss tablets available that can support weight reduction, provided they are deemed suitable, subject to clinical assessment where appropriate. Other non-prescription weight-management options are also available:  

• Orlistat 60mg capsules, such as Orlos or Alli brands, are pharmacy medicines that may be suitable for adults with a BMI of 28 kg/m² or above and are used alongside a reduced-calorie, lower-fat diet. Orlistat works by reducing the amount of dietary fat absorbed by the body 

• Formoline L112 is a non-prescription medical device in tablet form. It binds dietary fats in the gastrointestinal tract, reducing the amount of fat and associated calories absorbed by the body. 

Wegovy and Foundayo tablets are prescription-only medicines. They are only supplied where clinically appropriate after assessment by a clinician. Treatment is not guaranteed. Do not stop, combine or switch weight-management treatments unless advised by a clinician.

• A BMI of 27 or above with a weight‑related health condition (such as high blood pressure)

• The person is aged 18 or above

Alongside a reduced-calorie diet and increased physical activity, orforglipron is authorised for weight loss and weight maintenance in adults in the UK with a BMI of 30 or above, or a BMI of between 27 and 30, and at least one weight-related comorbidity. 

Foundayo is also authorised for the treatment of type 2 diabetes, but this is separate from its use for weight-management and is outside the scope of this weight-management service. 

A clinician will assess your individual circumstances, medical history and current medicines to determine whether any treatment may be suitable for you.  

Very common (more than 1 in 10) side effects of Foundayo weight loss tablets include:   

• Feeling sick (nausea) 

• Constipation 

• Diarrhoea 

• Being sick (vomiting) 

• Indigestion (dyspepsia) 

• Stomach (abdominal) pain 

• Low blood sugar (hypoglycaemia) when orforglipron is used with insulin with or without metformin and/or sodium-glucose co-transporter 2 inhibitors (SGLT2i). Your doctor may reduce your dose of insulin before you start using this medicine. The warning signs of low blood sugar may include headache, drowsiness, weakness, dizziness, feeling hungry, confusion, irritability, fast heartbeat and sweating. Your doctor should tell you how to treat low blood sugar 

Nausea, vomiting and diarrhoea are more common when starting treatment or increasing the dose and decrease over time in many patients. If you experience vomiting or diarrhoea, you may lose more fluid than usual resulting in dehydration. Make sure you drink plenty of fluids to stay hydrated. If symptoms are severe or persistent, contact a healthcare professional for advice. 

Common (may affect up to 1 in 10 people) side effects of Foundayo weight loss tablets include: 

• Low blood sugar (hypoglycaemia) when orforglipron is used with medicines that contain a sulfonylurea alone or combined with up to two additional diabetes medications for type 2 diabetes. Your doctor may reduce your dose of sulfonylurea before you start using this medicine  

• Dizziness 

• Headache 

• Fast pulse 

• Low blood pressure 

• Bloating of the stomach 

• Burping (eructation) 

• Reflux or heartburn (also called gastro-oesophageal reflux disease – GORD) - a disease caused by stomach acid coming up into the tube from your stomach to your mouth 

• Gas (flatulence) 

• Gallstones 

• Hair loss 

• Feeling tired (fatigue) 

• Increased levels of pancreatic enzymes (such as lipase and amylase) in blood 

Uncommon (may affect up to 1 in 100 people) side effects of Foundayo weight loss tablets include: 

• Low blood sugar (hypoglycaemia) when orforglipron is used without insulin or sulfonylurea. 

• Unusual sense of taste  

• Unpleasant and abnormal feeling when touched 

Uncommon (may affect up to 1 in 100 people) but serious side effects may include: 

• Inflamed pancreas (acute pancreatitis) which could cause severe pain in the stomach and back which does not go away. This is a serious, potentially life-threatening condition. You should see a doctor immediately if you experience such symptoms 

Serious side effects and symptoms requiring medical advice may include: 

• Non-arteritic anterior ischemic optic neuropathy (NAION) is a rare condition associated with the potential for decreased vision, including permanent loss of vision. Any sudden changes in your vision or any other unusual visual symptoms should be reported to your doctor straight away 

• Bowel obstruction which could cause severe tummy pain, a swollen tummy, being sick (vomiting) or not being able to pass wind or open your bowels 

• Severe constipation 

• Serious allergic reactions (anaphylactic reactions). You should get immediate medical help and inform your doctor if you experience symptoms such as breathing problems, swelling of face and throat, wheezing, fast heartbeat, pale and cold skin, feeling dizzy or weak 

• The chance of food or liquid getting into your lungs may increase during surgery or procedures requiring general anaesthesia or deep sedation 

• Kidney problems such as acute renal failure or worsening of chronic renal failure have been reported, sometimes requiring treatment with a kidney machine (dialysis). Signs may include passing less urine than usual, nausea and vomiting, feeling more tired than usual

You should tell your clinician if you:

Do not take Foundayo if you are allergic to orforglipron or any of the other ingredients in the medicine. 

You should speak to your clinician if any of the above apply to you, but also if you have any other conditions. Tell your clinician about all prescription medicines, over-the-counter medicines and herbal products you take. Some medicines can affect Foundayo, or be affected by it, so your clinician will check for interactions as part of your assessment.

Wegovy tablets may not be suitable, or may need additional clinical review, if you:  

• have kidney problems  

• are breastfeeding  

• have diabetic retinopathy  

• have low blood sugar levels (hypoglycaemia)  

• are pregnant or trying to get pregnant  

• have ever had a serious allergic reaction to semaglutide or any ingredient in Wegovy  

• have type 1 or type 2 diabetes  

• have a history of pancreatitis   

• have severe congestive heart failure  

• have gastroparesis 

• are undergoing anaesthesia or deep sedation

• feeling dizzy 

• burping

• feeling dizzy or lightheaded on standing or sitting up because of a drop in blood pressure 

• fast heartbeat 

• increase of pancreatic enzymes (such as lipase and amylase) shown in blood tests 

• a delay in the emptying of the stomach 

• low blood sugar (hypoglycaemia) in patients without diabetes 

• increased levels of bilirubin in your blood. Signs include jaundice which is yellowing of the skin or the whites of your eyes 

More serious, uncommon, rare, and very rare side effects may include: 

• acute pancreatitis can present as severe persistent abdominal pain, with or without nausea or vomiting – stop using the medicine and seek urgent medical attention 

• allergic reactions including anaphylaxis with symptoms like swelling, difficulty breathing, or severe rash – stop using the medication and call 999 immediately  

• kidney or bladder stones 

• hip fractures – seek medical attention after a fall where there is severe hip pain or an inability to stand or bear weight  

• sudden loss or rapid worsening of vision 

• severe constipation with abdominal pain, bloating or vomiting, or inability to pass stools or wind can be signs of bowel obstruction 

• severe upper abdominal pain with fever, vomiting or jaundice  

• significant dehydration, including inability to retain fluids, passing less urine, faintness, or confusion

Wegovy tablets are taken once a day. They are swallowed whole and cannot be split of crushed. You should take Wegovy tablets first thing in the morning, on an empty stomach after a recommended fasting period of at least 8 hours, with a small sip of water (the manufacturers advise no more than 4oz, which is about 100ml). Wegovy tablets should be taken 30 minutes before you eat, drink, or take any other medicines.

Foundayo can be taken once daily at any time, with or without food and without water restrictions. It should be swallowed whole and not broken, crushed or chewed.

You can take the maintenance dose agreed with your clinician for as long as it’s helping, as long as you’re not experiencing any side effects (or if any common side effects are being managed), and as long as it’s clinically safe. While using the medicine, it’s important that you keep in touch with your clinician or provider to stay on top of your progress or any issues you are having. 

If you experience an increase in weight, you should include this on your next consultation with a clinician for your repeat treatment so that they can review your dosage. If you wish to discuss stopping your maintenance dose, you should speak to your clinician, who will advise whether this is clinically appropriate for you. If so, they can work with you to develop a structured plan for stopping treatment and support you in maintaining your weight loss through longer-term lifestyle changes, including diet and physical activity.   

If you wish to restart treatment, you need to submit a consultation form. A clinician will review your full treatment and medical history to determine whether restarting medication is safe and appropriate. All prescriptions must align with current UK prescribing guidelines, which require a clinical need based on your present eligibility (e.g., BMI and health risks). Your clinician will ensure any decision prioritises your safety and long-term weight management goals.

Wegovy tablets have been shown to be effective for weight loss when compared with a placebo (a tablet with no active medicine). In OASIS 4, the EMA reported average weight loss of 13.61% with Wegovy tablets compared with 2.18% with placebo over 64 weeks. Novo Nordisk has separately reported 16.6% mean weight loss when treatment was adhered to compared to 2.7% with placebo over the same 64-week period. Individual results vary. 

Foundayo tablets have been shown to be effective for weight loss when compared with a placebo (a tablet with no active medicine). In ATTAIN-1, participants without diabetes receiving the highest equivalent dose of orforglipron who remained on treatment lost an average of 12.4% of their body weight over 72 weeks, compared with 0.9% for placebo. Participants also received diet and physical-activity support. Individual results vary. 

GLP-1 tablets such as Wegovy are prescription-only medicines that have been assessed by regulators for safety, quality and effectiveness. They are not suitable for everyone and should only be used under clinical supervision. 

The safety of the medication depends on several factors, including having a consultation to ensure you're a suitable candidate, following the correct dosage schedule, and regular monitoring by a clinician. This gives you professional guidance and support should you need it, such as managing side effects or issues with your medication.  

However, the popularity of GLP-1 medications has increased, which has coincided with the increase of illegal and falsified versions of the medicine. These may be falsely advertised online and on social media. Taking unauthorised GLP-1 medication can cause serious or unexpected side effects and may interact dangerously with any other medication you’re taking. GLP-1 tablets are only available on prescription and should only be taken under the supervision of a healthcare professional. Never buy them without a prescription or from unverified sources. 

There is no single ‘best’ tablet, or treatment of any kind, for weight loss. The most suitable weight loss treatment for you depends on your individual health profile, your BMI, and your clinician’s assessment of your consultation form.

Although Wegovy tablets and Wegovy injections contain the same active ingredient, they are taken and absorbed differently. Orforglipron and semaglutide are both GLP-1 receptor agonists, but they have different molecular structures and are absorbed and processed differently. Their dosing and administration instructions therefore differ. 

This means dosing, instructions and suitability may differ. A clinician can advise which formulation, if any, may be appropriate for you. The effects and clinical outcomes of each formulation may vary between patients.

When you stop taking your weight loss medication, the effect of the medication will gradually fade. Appetite and hunger may increase, and the feeling of fullness after eating may reduce. Gastric emptying returns to its previous rate, which can mean feeling less full after meals. 

This means the likelihood of putting the weight back on you have previously lost increases. One study from 2022 found that after 68 weeks of weight loss treatment, the average patient saw a drop of 15% in their body weight, but within one year from the end of their treatment, patients saw an average of two thirds of their prior weight loss returned.

Whether you’ve decided that you’d like to switch from Mounjaro to Wegovy, from Wegovy to Foundayo, or from injections to tablets, you’ll want to know what happens next. You will need to schedule a consultation to speak with one of our clinicians, who will guide you through the process, review your medical history, and ensure that changing your weight loss medication is the right decision for you. If suitable, they will put together a transition plan for you.

A two‑month supply allows you to receive two months of your clinically approved weight loss plan from a single consultation with your clinician, subject to their review of your eligibility and suitability. 

Eligibility for a two‑month supply is based on a full clinical assessment, which includes verification of your weight and BMI, as well as ensuring you are already using a medicated weight loss service, whether with Pharmacy2U or another provider, for at least one month and are on (or due to be on) a maintenance dose. If you’ve used another provider previously, the clinician will need to verify your treatment history before approving a two‑month supply 

It is available on both Mounjaro and Wegovy, except the 7.2 mg dose of Wegovy. 

You get two of the same doses of your clinically approved weight loss plan, which equals two-month's supply, subject to a full consultation with your clinician.

The two-month supply service is only available to existing weight loss patients who have completed one full month of treatment, whether with Pharmacy2U or another provider, and are on or due to go on to a maintenance dose (as you can only request two of the same doses rather than two different doses). If you’ve used another provider previously, the clinician will need to verify your treatment history before approving a two‑month supply. 

Your clinician will solely determine whether a one or two‑month supply of your clinically approved weight loss plan is appropriate based on your treatment plan. 

Even if you are eligible and your clinician has agreed that you can order a two-month supply, if you still only wish to order one month’s supply at a time, you can continue to do so.

A two-month supply is not a clinical option for new Online Doctor patients who are not currently on a medicated weight loss plan. It may be an option for existing Pharmacy2U Online Doctor weight loss patients and patients joining Pharmacy2U directly from another medicated weight loss provider. However, if you have used another provider previously, the clinician will need to verify your treatment history before approving a two‑month supply and your eligibility is solely determined by them.
