To use our Prescription Delivery Service, follow these simple steps:
1. Print out the following order form:
in Word for Windows format
in Acrobat format
2. Please complete your payment details and contact telephone numbers on the form.
3. Check that the address on your prescription is correct and includes your postcode. If you wish to have your medicines delivered to an address other than the address on your prescription, simply add the details in the appropriate section of the form. Please be aware when choosing your address that a signature will be required on delivery.
4. Post the completed form ALONG WITH YOUR PRESCRIPTION to:
Pharmacy2U Ltd, Freepost RLTU-BHYL-KEZJ, Leeds LS14 1PQ.
Freepost addresses only allow second class delivery. If you would like us to receive your prescription sooner, please attach a first class stamp and send to: Pharmacy2U, 1 Hawthorn Park, Coal Road, Leeds LS14 1PQ.
5. Please allow 5 working days from the date you post your prescription for your medicines to be delivered.
